With sending this form I agree to:

-start building a relationship with my coach to work towards my goals,

-allow my Coach to contact me and send me informational materials as well as "homework",

-commit to at least 4 weeks to this process,

-to inform my Coach if something is bothering me or doesn't work for me,

-pay the coaching fees before the coaching session starts.
