I understand that the coaching services provided by mindfulmess mom LLC are not a substitute for medical or psychological care, and that the coach is not a licensed mental health professional in the State of California. I acknowledge that the coach is not responsible for diagnosing or treating any medical or psychological conditions.

I am aware that participating in coaching sessions may involve physical, emotional, and mental exertion, and that there are inherent risks involved with any physical or mental activity. I understand that these risks include, but are not limited to, physical injury, emotional distress, and mental anguish.

In consideration of being allowed to participate in coaching sessions, I hereby waive, release, and discharge any and all claims, demands, causes of action, and liability arising from or related to my participation in coaching sessions. This waiver, release, and discharge applies to any and all claims, whether known or unknown, arising from or related to my participation in coaching sessions.

I hereby assume all risks associated with my participation in coaching sessions, including but not limited to physical injury, emotional distress, and mental anguish. I understand that the coach does not guarantee any specific results from participating in coaching sessions.

I agree to indemnify and hold harmless mindfulmess mom LLC and its employees, agents, and affiliates from any and all claims, damages, costs, or expenses, including reasonable attorney fees, arising from or related to my participation in coaching sessions.

I have read this liability waiver, express assumption of risk, and release of liability and fully understand its terms. I understand that by agreeing to this document, I am giving up substantial rights, including my right to sue. I agree that this document is binding on me, my heirs, executors, administrators, and assigns.

This liability waiver, express assumption of risk, and release of liability is governed by the laws of the State of California. Any disputes arising from this document shall be resolved in accordance with the laws of the State of California.

By using the coaching services provided by mindfulmess mom LLC, I acknowledge that I have read, understood, and agree to this liability waiver, express assumption of risk, and release of liability. If I do not agree to this document, I will not participate in coaching sessions provided by mindfulmess mom LLC.

You are the parent and/or lawful guardian of the child or Teacher.

ACTING ON BEHALF OF YOURSELF, AND TO THE EXTENT ALLOWED BY LAW, YOU HEREBY WAIVE AND AGREE TO RELEASE ANY CLAIMS OR LIABILITY WHICH YOU, YOUR CHILD, OR YOUR CHILD’S HEIRS AND SUCCESSORS MAY HAVE OR BELIEVE TO HAVE AGAINST mindfulmess mom, LLC AND ITS OFFICERS, EMPLOYEES, OR AGENTS FOR ANY INJURIES, LOSSES OR DAMAGES TO YOUR CHILD, YOUR CHILD’S PERSONAL PROPERTY AND YOUR PERSONAL PROPERTY.

YOU AGREE TO BE RESPONSIBLE FOR, INDEMNIFY, AND HOLD HARMLESS mindfulmess mom, LLC FROM AND AGAINST ANY CLAIMS, SUITS, JUDGMENTS, OR COSTS WHICH MAY BE BROUGHT AGAINST mindfulmess mom, LLC, ITS OFFICERS, DIRECTORS, EMPLOYEES OR AGENTS FOR THE ACTUAL OR ALLEGED ACTS OR OMISSIONS OF YOU OR YOUR CHILD.

I have read the above Liability Waiver, Express Assumption of Risk, and Release of Liability in its entirety and I freely and voluntarily assume all risks of such Injuries and Damages and notwithstanding such risks, I agree to participate in the services and workshops provided by mindfulmess mom LLC.
